What to bring to your eye exam

  • Alberta Health Care Card
  • Private insurance card / information. Please be prepared and check your vision coverage before coming for your eye examination so you are well informed.  
  • Photo ID (needed to confirm identity for insurance billings)
  • A list of all medications that you take, including vitamins, supplements, and drops.
  • Bring your glasses so we can compare your new prescription to the prescription in your glasses.
  • Bring your contact lens packages or boxes so we know the type, size, and prescription of your contact lenses that you wear.
  • Any forms that you require to be filled out after your eye examination. Extra fees may be required depending on the length and requirements of the form.

Privacy Policy

Please see our privacy policy document. You will be asked to sign a patient agreement when you arrive in our office for the first time, which will include your agreement to our privacy policy.

In-Take Forms

We will ask you to fill out a medical and ocular history questionnaire when you check-in for your first appointment at Jagare Ridge Vision Care & Development. We ask that if you would like to save time upon check-in, that you have this form filled out, and either printed or emailed to us ahead of time.

Personal and medical history is very important in allowing your Doctor of Optometry to provide the most comprehensive care by understanding you as a person rather than just your eyes. If you have any questions, please save them to ask our staff when you check in.
